Preparing for your Appointment
To ensure a smooth and efficient visit, please bring the following items:
- Insurance Card
- Photo Identification (e.g., Driver’s License)
- Current Medication List: Include both prescription and over-the-counter medications. Note the dosage and frequency
- Recent Test Results and Imaging: Lab reports, MRI films, or any other relevant studies
- Medical History Records: Important details about your past medical and surgical history
