Preparing for your appointment

Please bring the following for the visit
  • Your insurance card 
  • A list of the current prescription and over-the-counter medications you are taking, including the dose and frequency 
  • All recent Lab reports , MRI studies (films) and other medical records related to the visit
  • All other important information about your medical and surgical history
